At CNY Healing Arts Centers we are always mindful of making things easier on our clients. With that mission in mind, we now have our Consent Forms available to download here. This way you can print them out at home, fill them out when it’s convenient to you and bring the form with you to your appointment. You will need the ability to open a Word document.
